- Next time you get a school or sports physical, have your doctor fill this out too !!!!!!
- Starting 2011, Owasippe will only accept physical exams on this form !!!!!!!!!!!
- Required annually for Summer Camp, High Adventure, Order of the Arrow, etc.
- Also required annually for adult volunteers spending more than 72 hours at a scouting event.
On-line Youth Protection Training
- Required every two years – all adult volunteers.
